Consumer Health Data Privacy Policy

This policy describes how KAYU Health (“KAYU”) handles consumer health data, as required by the Washington My Health My Data Act (RCW 19.373) and similar laws. It is separate from, and supplements, our general Privacy Policy.

Categories of Consumer Health Data We Collect

Through this website we may collect the eligibility-routing answers a visitor provides in the intake questionnaire — including a general interest area and state of residence — together with the fact that a visitor used this site to seek information about, or to request, health-related services. We do not collect symptoms, diagnoses, medications, or clinical history through this site before a consultation.

Purpose of Collection

We collect this consumer health data solely to route a request to an appropriate program, to schedule and deliver a requested consultation, and to communicate with the visitor about that request.

Sources of the Data

We collect this consumer health data directly from the visitor through the intake form and questionnaire on this website. We do not purchase or acquire consumer health data about visitors from third parties.

Categories of Third Parties and Affiliates We Share With

We share consumer health data only with service providers and affiliates that help us deliver the requested service, and only as needed. The categories of recipients are our customer-relationship and booking platform and the licensed clinical provider entity that reviews the request. The specific affiliates with whom consumer health data is shared are:

KAYU Health (operator of this site); the licensed medical group affiliated with KAYU that reviews consultation requests; HighLevel / LeadConnector, our customer-relationship and booking platform; and Supabase, our database provider, operating under a signed Business Associate Agreement.

We do not sell consumer health data, and we do not share it with advertising partners.

How to Exercise Your Rights

A consumer has the right to access their consumer health data, to withdraw consent to its collection and sharing, and to request that it be deleted. To exercise any of these rights, contact us at privacy@kayuhealth.com. We will respond to verified requests as required by applicable law.
